General information

Below is a brief summary of the information you need before attending your appointment:

New Patients

Please complete the patient form prior to your first appointment as this will enable us to prepare your file before your arrival.
We would appreciate the return of your completed form via e-mail (drajford@gmail.com) or fax to (012) 361 7350.

Please note: We will not make any claims from your medical aid on your behalf. You are solely responsible for settling the account immediately after consultation and before receipt of medication, where after you may claim the paid amount from your medical aid.

We assure you that all our codes (ICD-10, NAPPI (where applicable) and Tariff codes) are up to date, according to the BHF, and indicated on the statement. For purposes of our administration, the main member’s initials and surname is used. Please quote the main member's particulars with all enquiries.

Consultation

All consultation fees exclude any medication (if prescribed) and are subject to change without prior notice.

Please note: Every patient consulting with the doctor must make a separate appointment. The duration of the consultation is 15 minutes and any further consultation thereafter will be charged for accordingly.

Method of payment

• Cash (ATM available in the Glenfair Shopping Centre next to us)
• Internet transfer
• Credit / Debit card facilities are available for transaction amounts larger than R500.
• Regretfully we do not accept cheques

Cancelation of appointments

Cancelations must be made telephonically 24 hours prior to the appointment on (012) 361 1075. 
Patients will be held liable for the consultation fee of appointments not cancelled at least 24 hours in advance.

Repeat Scripts

Orders for repeat scripts should be placed the day before collection.
You may only place your order telephonically or via email.

New patients: Scripts may be repeated for 3 months, if applicable. A follow-up appointment must be made before further scripts will be given.
Existing patients: Patients that are being treated for age related or chronic illnesses  ( i.e. blood pressure, diabetes, cholesterol, hormones, arthritis, osteoporosis, heart disease, asthma, sinus, etc) must attend a follow-up appointment every 6 months.

Generally: Should you fail to make and attend appointments as herein described, and you continue taking any product prescribed to you by this practice, you do so entirely at your own risk and thereby forfeit any right to claim damages from Dr AJ Ford Incorporated, its directors or employees.

Placing your order  

Telephonically: Please call (012) 361 1075 to place your order the day before you wish to collect.
By email: Please email your order to drajford@gmail.com the day before you wish to collect.

Ensure that your email clearly states:
- Main member's initials and surname
- The name of the patient for whom the medication is prescribed.
- A list with names and dosages of the medication required.
- Method of collection: Collect from Practice; Courier to collect; Post via Postnet. (If you would like us to send your parcel through Posnet (not the Post Office) please clearly state the closest Postnet to your area)

Collection of scripts

Parcels may be collected from 8h00-16h00 Mondays, Tuesdays, Thursdays and Fridays and 8h00-15h30 on Wednesdays.
Should you have any queries regarding your script please feel free to e-mail or call us.

Posting of medication

For patients who cannot collect their medication from the Practice we offer a posting service through Postnet at an additional charge.
We no longer make use of the SA Post office for posting of medicine.
Orders for scripts must be placed and paid by 13:00 the day preceding postage.
You may phone back after 1 hour of placing your order to confirm the amount and retrieve your tracking number.
We do not post parcels that have not been paid for.

The practice does not accept any responsibility for the parcel once it has been collected by Postnet.

The telephone number for tracking your parcel is 012 365 2664/5. Please have your tracking number closeby.

Returns

Under no circumstances may any prescription be returned.
